The best system lasted eleven days
The best system I've ever used to track tasks across multiple tools was a spreadsheet. It was clean, logical, pleasingly color-coded, and it lasted about eleven days before reality ate it alive – which, as far as I can tell, is roughly the universal half-life of any manual tracking system, regardless of how smart the people maintaining it are or how many conditional formatting rules they've lovingly applied.
We had columns for the Linear ticket, the GitHub PR when there was one, a link to whatever Notion doc held the context, and a status field that was supposed to reflect what was actually happening. All perfectly reasonable, and completely abandoned within two weeks, because nobody on a six-person team wants to context-switch out of their actual work to go update a spreadsheet that only exists because their tools can't keep track of themselves. The whole exercise – doing work about tracking work – was eating roughly half an hour per person per day, which adds up to something genuinely depressing if you bother to do the math across a quarter. We were, in effect, paying a full-time employee's worth of hours just to maintain a document that was already wrong by the time anyone checked it.

I believed the myth for a while. We tried several of these tools (I won't name them all, but if you've been down this road you've probably tried a few of the same ones), and they all shared the same fundamental limitation: they were still single tools. A dashboard that aggregates your GitHub data alongside your Slack threads and your Notion pages is better than a spreadsheet, sure, but it's still imposing its own model of what a "task" is and trying to force-fit everyone else's model into its schema. The information gets flattened, the relationships get lost, and what you end up with is a very expensive read-only view of data you already had access to, presented in a layout that doesn't quite match how any of the source tools organized it in the first place.
And here's the recursive part that I find almost comically perfect: a "single pane of glass" that requires you to set up integrations, configure mappings, maintain yet another dashboard, and check yet another app is not reducing your tool sprawl – it's adding to it. You now have n+1 tools instead of n, and the n+1th tool's entire job is to watch the other n, which means its accuracy degrades in direct proportion to how many tools it's watching and how often those tools change their APIs. We have too many tools, so we adopt a tool to manage the tools, and when that tool doesn't quite work we adopt another tool to fill in the gaps left by the tool that was supposed to fill in the gaps. At some point you step back and realize you've built a Rube Goldberg machine of SaaS subscriptions, and the actual work – the thing all these tools were supposed to serve – is happening in spite of the tooling, not because of it.
The myth is that this is a visibility problem – that if you could just see everything in one place, you'd be fine. The mechanism is that it's actually a problem of relationships. No single tool can track tasks across multiple tools because each tool has its own model of what a task is, and those models are fundamentally incompatible. A dashboard that displays them side by side doesn't make the models compatible. It just makes the incompatibility prettier.

What each tool actually sees
Let me walk through this concretely, because I think the abstraction hides how bad the situation actually is.
Take a single piece of work – implementing a new API endpoint, say. In Linear, that's an issue with a status, an assignee, a priority, and a cycle. In GitHub, it's a PR (or maybe two – one for the backend, one for the client) with a review state, CI checks, and a merge status. Over in Slack, it's a thread where someone asked a question about the approach and three people debated it for forty messages before arriving at a completely different design. In Notion, there's an RFC page that two people contributed to and one person forgot to update after the Slack conversation changed everything. And somewhere in Figma, a comment on the original design triggered the whole change in the first place.
That's five tools, one task, and zero of those tools have any idea that the other four are talking about the same thing. The Figma comment doesn't know the RFC exists. The Slack thread doesn't know there's a ticket. GitHub doesn't know the approach changed. Each tool tracks its own domain beautifully – the problem is that no single tool sees the full lifecycle of a task that spans multiple tools, and at a team our size, basically every task that takes more than a day does exactly that.
Human memory is the bridge between all of these islands, and human memory (as anyone who's ever missed a Slack thread while on a call can tell you) is a depressingly finite resource to build your entire project visibility on.
The three ways tasks vanish
After watching cross-tool tracking break down across dozens of tasks (and, honestly, contributing to a fair number of those failures ourselves), we started to see patterns. There are really three distinct failure modes, and I think naming them is useful because they require different fixes.
The ghost task. Work exists in one tool but never surfaces in the others. Someone files an issue, the related PR happens without anyone linking back to it, the discussion about the approach happens in a channel the issue creator isn't in, and three weeks later the task looks blocked to everyone except the person who quietly shipped it and moved on. The work got done – nobody knows.
The stale status. A task's status in one tool drifts out of sync with reality because the actual progress is being tracked elsewhere. The ticket still says "In Progress" but the PR merged yesterday. The doc says "Draft" but the team already approved a different approach in a thread nobody bookmarked. Anyone who checks the supposed source of truth gets the wrong picture, and decisions get made on stale data – which is, in some ways, worse than having no data at all, because at least with no data you know you're guessing.
The orphaned context. This one is the subtlest, and (in my experience at least) the one that causes the most actual damage. A conversation happens that changes the direction of a task – maybe a constraint nobody anticipated, maybe a better approach someone thought of – but that conversation never gets reflected back into the original spec. Two weeks later, someone picks up the task based on the original requirements, builds the wrong thing, and the team loses a sprint's worth of work. The context existed the entire time – it just lived in a tool that the task didn't know about.
All three failures have the same root cause: the tools don't share a model of what's happening. They're islands with human-attention bridges, and human attention is exactly the resource that's always in short supply.
What you can do right now (without buying anything)
Before I get into the systems-level fix (and I promise I'm not building to a sales pitch – well, not entirely), there are a few things that genuinely help reduce cross-tool tracking failures using nothing but discipline and some lightweight process changes. We tried all of these before building anything, and some of them still matter even with better tooling.
Designate a canonical home for every task. Pick one tool as the source of truth for status (for us it's Linear) and make a team rule that any status-changing decision gets reflected there within 24 hours, no matter where the conversation happened. This doesn't solve the problem, but it reduces the stale status failure mode significantly.
Create a weekly orphaned-context sweep. Once a week, have someone (rotate it) scan the last week's Slack threads and check whether any decision or direction change got captured in the relevant ticket or doc. Fifteen minutes of intentional bridging catches more dropped context than you'd expect.
Use cross-links religiously. When you open a PR, link the issue. When you start a Slack thread about a task, drop the ticket URL in the first message. When you update a doc, mention it in the thread. This is boring and manual and nobody wants to do it (which is why it degrades over time), but while it works, it works well.
Set a stale-status SLA. If a ticket hasn't been updated in five business days and there's been activity in the related PR or thread, flag it. This can be as simple as a weekly Linear filter someone eyeballs.
None of these are permanent solutions – they all depend on humans remembering to do things, which is the exact resource we've established is unreliable – but they meaningfully reduce the damage while you figure out whether the problem is bad enough to justify a structural fix.
What a real fix looks like (and what we're still figuring out)
I want to be careful here, because I've just spent several paragraphs being sardonic about tools that promise too much, and the last thing I want to do is turn around and make the same kind of promise. So let me describe what we think a real fix looks like, with the honest caveat that we're still working through some of this ourselves.
The key insight – and I realize this sounds obvious once you say it, but it took us a while to get here – is that you don't need another dashboard. You need a knowledge graph. Not a read-only aggregation of data from your tools, but something that actively understands the relationships between items across them.
