- Information loss at handoff. The client's format requirement existed in an email attachment that didn't survive the transition through Slack to Linear. By Wednesday, the engineer was working from memory, not from the original spec.
- Ambiguous ownership of delivery. Neither the engineer nor the PM had explicit ownership of the final send-to-client step.
- No verification between "done in tracker" and "done in reality." The Linear status was treated as ground truth, but it only reflected engineering completion, not full delivery.
Each of these is fixable without a new process document that everyone agrees to read and nobody actually does. The fixes are about making connections between existing tools more explicit:
