Every project management tool on the market promises to be the place where nothing gets lost, which is an interesting pitch given that the average team now uses six or seven of these things simultaneously, each one solemnly swearing it's the single source of truth while the actual truth is distributed across all of them and faithfully recorded in none. Tasks falling through the cracks isn't a failure of any one tool – it's a natural consequence of spreading work across tools that have no idea the others exist.
That's not really a software problem. It's a species problem.
The anatomy of one dropped task: a forensic timeline
I want to trace one specific task that fell through the cracks on a team I worked with last year – not because it was dramatic, but because it was so ordinary that nobody even noticed the drop until it had already cost the team about a week.

Three people discuss it, agree it should be fixed before launch, and someone (it's not clear who, which is part of the problem) says they'll "make sure it's tracked."
Tuesday, 9:15 AM. Standup. Nobody mentions the contrast issue. It's not in Linear, so it doesn't appear on anyone's board. The designer assumes the engineer filed the ticket. The engineer, who is now deep in an unrelated refactor, has genuinely forgotten. The QA engineer assumes the Slack thread handled it. Everyone's assumption is perfectly reasonable and completely wrong.
Thursday, 4:00 PM. The release ships, and the contrast issue ships with it. A customer with low vision reports it through support three days later, and while the actual fix takes an engineer about twenty minutes, the surrounding mess – the support ticket, the escalation, the retrospective conversation about how this got missed, the pull request with its apologetic commit message – takes closer to a day.
Friday, retrospective. The team agrees they need "better ticket hygiene." Someone suggests a Slack bot. Someone else suggests a weekly triage meeting. These are both perfectly sensible ideas that address approximately none of what actually happened.

The real villain isn't the tooling
If you look at the timeline, the signal existed the entire time – in Figma on Monday morning, in Slack by Monday afternoon. Three separate people identified the same issue, discussed it, and agreed it mattered. The information was correct, visible, and completely useless, because it never made the jump into the one place where visibility translates into action.
Your task tracker has a fundamental limitation that rarely gets discussed in its marketing materials: it can only track work that someone has already typed into it. The Figma comment doesn't exist in Linear's universe. The Slack conversation where three people decided something should happen doesn't exist there either. Each tool is a closed system with excellent internal search and absolutely no awareness of what's happening next door. It would be comforting if the fix were just "better integrations," because that's a problem you can buy your way out of. But the engineer who said "I'll file a ticket" wasn't careless – he got pulled into a PR review that required his attention, and the Figma comment didn't have a snooze button, so it relied entirely on his memory to survive the context switch. The QA engineer who said someone would "make sure it's tracked" wasn't being vague out of negligence – in Slack, saying "someone should track this" feels like a concrete action even though it delegates to nobody in particular. I've seen teams try to bridge these gaps with intake forms, Slack-to-Jira bots, mandatory standup questions about "anything not yet ticketed?" – and honestly, some of them work for a while (we ran a Slack bot for about three months before people started reflexively ignoring it, which is the eventual fate of every automated nag).
The uncomfortable truth (and I'm not sure there's a clean fix for this, to be honest) is that things falling through cracks at work is mostly a consequence of how human attention works when it's spread across too many channels. We're inconsistent creatures – distractible, tired, subject to the bystander effect – and no amount of discipline training overcomes the fact that you switched contexts thirty times today and each switch cost you a little bit of whatever you were holding in your head.
The gap between “someone identified the problem” and “someone tracked the problem” is where most dropped balls live. That gap is a human attention problem, not a tooling problem, which is why adding more tools rarely closes it.
What actually helps (with honest caveats)
Here are four practices that cost nothing and make a genuine difference. I'll be upfront about where each one tops out, because pretending any of these is a complete solution would be dishonest.
Rotating signal owner. One person per team, rotated weekly, whose entire job is to scan Slack threads and meeting notes for things that should be tracked but aren't. This works remarkably well when it's in place, because it converts the bystander problem into an explicit assignment – one person owns the gap. It tops out because the signal owner can't monitor Figma comments, PR review threads, and email simultaneously (well, they can try, but it becomes a full-time job pretty quickly).
24-hour triage SLA. Anything flagged as potentially actionable gets sorted within a day: turned into a ticket, linked to an existing one, or – and this is the part most teams skip – explicitly dismissed with a reason. That dismissal matters enormously. It means someone looked at the signal and decided "no." Way better than letting signals float, unacknowledged, in perpetuity.
Emoji tagging in Slack. We use a ticket emoji to mean "this needs a ticket." Anyone can tag any message, takes two seconds. The signal owner checks tagged messages each morning. It's embarrassingly low-tech and annoyingly effective, right up until someone tags a message at 4:55 PM on a Friday and nobody checks until Monday.
PR review checkpoint. Before merging: "Did any comments in this review need to become tickets?" One question, maybe ten seconds. Catches the refactoring warnings and the "we should really fix this later" notes that otherwise vanish into GitHub's bottomless archive.
These are all good habits and I'd recommend every one of them. But they share a common ceiling: they rely on humans remembering to do a thing consistently, and (here's the species problem again) we just don't. You'll catch more drops. You won't catch all of them.

Let me circle back to the accessibility issue from the forensic timeline, because the downstream cost is worth spelling out.
The engineering fix took twenty minutes. The total cost – support ticket, customer escalation, internal investigation, retrospective, and the PR to fix it – was closer to a full day of work spread across three people. One dropped signal, maybe six hours of waste. That math doesn't look terrible in isolation, but in my experience a team of eight to ten people drops three or four signals per sprint, which adds up to something like six to eight hours of wasted time every two weeks.
The harder cost to quantify (and I suspect the more expensive one) is the ambient background anxiety – that low-level hum of "am I forgetting something?" that every engineer on a multi-tool team carries around. The over-checking, the DMs that say "hey, just confirming we didn't lose track of the thing from Tuesday," the status meetings that exist solely because nobody trusts the system to hold context. That's cognitive overhead that doesn't show up in any sprint report but absolutely shows up in how people feel about their work.
